You have to cut the tip open which makes it a pretty large opening. You're more like splashing this in their ear than squirting it. I did it, it says to do liberally, so I really poured it in there, and my cat, naturally, shook his head immediately, shaking all of that liquid out at me and the wall. Of course he would do that naturally, I just didn't expect it! It's best to sneak up on them while they're sleeping, pour it in, and hold the ear flap down for the massage. They will shake some out, but it's to be expected.
